An occupational health report is written based on the information obtained from the consultation with the occupational health practitioner who sees you and any supporting medical evidence provided. A certificate rather than a report is provided for outcome of fitness assessments or health surveillance. The report may be sent to one of the following:

  1. Directly to your employer (usually your line manager and to Human Resources)
  2. A third party occupational health provider who will forward the report onto your employer.

The content of the report will be discussed with you as part of the consultation. It will include answers to any questions that your manager/Human Resources has asked on the referral. Third party occupational health providers will maintain your occupational health record with both clinical notes and reports that are written for your employer.

Access to the report
You may request to see the occupational health report before or at the same time as it is sent to your employer. If Insync Corporate Healthcare are responsible for sending the report to your employer, they will send a copy of the report either by e-mail to the e-mail address you supply or by post to the address you confirm at the time of the appointment.

If the report is sent to a third party provider, it may be edited and the provider is responsible for sending the report to your employer. They will send you a copy of the final report either at the same time or in advance of your employer depending on your choice directed on the consent form.

Third party providers are required to maintain the same level of confidentiality as Insync Corporate Healthcare.

Requesting changes to the report or refusing consent to send the report
The report written by Insync Corporate Healthcare contains an objective assessment of fitness for work based on the information provided. You are entitled to raise your concerns within the time frame stated on the consent form. The author of the report may agree to amend the report if there are factual errors within the report or you may withdraw your consent for its distribution. You may also attach a letter outlining your opinion/comment to the report.

Mae adroddiad iechyd galwedigaethol yn seiliedig ar wybodaeth sy’n deillio o’r ymgynghoriad â’r meddyg iechyd galwedigaethol a welsoch chi, ac unrhyw dystiolaeth feddygol ychwanegol. Darperir tystysgrif yn hytrach nag adroddiad am ganlyniadau asesiad ffit i weithio neu wyliadwriaeth iechyd. Efallai y bydd yr adroddiad yn cael ei anfon at un o’r canlynol:

  1. Yn uniongyrchol i’ch cyflogwr (fel arfer eich rheolwr llinell a’r adran Adnoddau Dynol)
  2. Darparwr gwasanaeth iechyd galwedigaethol trydydd parti, a fydd yn anfon yr adroddiad ymlaen at eich cyflogwr.

Trafodir cynnwys yr adroddiad gyda chi fel rhan o’r ymgynghoriad.  Bydd yn cynnwys atebion i unrhyw gwestiynau a ofynnodd eich rheolwr/adran Adnoddau Dynol pan wnaed yr atgyfeiriad. Bydd darparwyr Iechyd Galwedigaethol trydydd parti yn cadw eich adroddiad Iechyd Galwedigaethol ynghyd â nodiadau clinigol ac adroddiadau ysgrifenedig at sylw eich cyflogwr.

Eich hawl i weld yr adroddiad
Gallwch ofyn am weld yr adroddiad iechyd galwedigaethol cyn iddo gael ei anfon at eich cyflogwr neu yr un pryd ag y caiff ei anfon at eich cyflogwr. Os mai Insync Corporate Healthcare sy’n gyfrifol am anfon yr adroddiad at eich cyflogwr, byddant yn anfon copi o’r adroddiad i’r cyfeiriad e-bost a ddarparwyd gennych, neu’n ei bostio i’r cyfeiriad a gadarnhawyd gennych adeg yr apwyntiad.

Os anfonir yr adroddiad at ddarparwr trydydd parti, efallai y caiff ei olygu a’r darparwr sy’n gyfrifol am anfon yr adroddiad at eich cyflogwr. Bydd yn anfon copi o’r adroddiad terfynol atoch yr un pryd ag y caiff ei anfon at eich cyflogwr, neu cyn hynny, yn dibynnu ar eich dewis ar y ffurflen gydsynio.

Mae’n ofynnol i ddarparwyr trydydd parti sicrhau’r un lefel o gyfrinachedd ag Insync Corporate Healthcare.

Gwneud cais am newidiadau i’r adroddiad neu wrthod cydsynio i anfon yr adroddiad
Mae adroddiad ysgrifenedig Insync Corporate Healthcare yn cynnwys asesiad gwrthrychol o p’un a ydych yn ffit i weithio ai peidio yn seiliedig ar wybodaeth a gyflwynwyd. Mae gennych hawl i fynegi eich pryderon o fewn y terfyn amser a nodir ar y ffurflen gydsynio. Gall awdur yr adroddiad gytuno i ddiwygio’r adroddiad os bydd gwallau ffeithiol ynddo neu gallwch wrthod cydsynio i’w ddosbarthu. Gallwch hefyd atodi llythyr yn amlinellu eich barn/sylwadau am yr adroddiad.
